Cardiac consultation-shortness of breath

One of the most commonly diagnosed cardiac symptoms is the symptom of shortness of breath. This symptom can be evaluated in our clinic with consultation, cardiac examination, EKG, and cardiac monitoring to determine the cause of symptom. The cost of cardiology consultation for evaluation of shortness of breath symptoms usually starts at $125.00 for new patients. This offer allows for uninsured, new patients to receive a $50.00 discount for out-of-pocket expense if appointment is booked online. Therefore, uninsured patients will pay $75.00 for online booking. Discount does not apply to testing, imaging, or procedures, i.e. labs, ECG, stress testing, echo, angiograms, etc., if needed. (Insured patients: we accept most insurance company cards for coverage of all medical expenses in insured patients, leaving insured patients with minimal, if any, out-of-pocket co-pay or cost.) Please bring a list of all medical conditions, surgeries, medication and supplement list, medical allergy list, recent lab results, i.e. CBC, CMP, lipid panel, TSH, Vitamin D, CRP, calcium score (if available), cardiac testing reports, i.e. ECG, stress test, echo, angiogram/cath, PCI (if available), name and contact information for primary care physician, including fax number (if available). This will assist in continuity of care and providing a comprehensive cardiac evaluation. There is a $25.00 nonrefundable fee for missed appointments if the patient does not reschedule or cancel the appointment within 48 hours of appointment date and time.
